关于BQ24172充电问题

针对两节8.4V锂电池充电，输入12V的适配器。

原理图如下：

 

PCB图如下：双面板，单面放器件

可以正常使用，但是经常容易坏，输出只剩下0.8V，重新换芯片可以正常使用。什么问题会超成这种这种情况，目前已进入量产，但是这个不稳定，超成很多不良率。
